PRIMETIME HEALTH A Special Level of Care Crossroads Hospice and GMC team up to support fragile family members A s part of Gwinnett Medical Center’s efforts to transform healthcare for seniors, it has added on-site hospice care as a special service for fragile patients. True to form, GMC sought out the best of the best to provide this level of care to patients and their families, and after much careful consideration, it selected Crossroads Hospice. Crossroads now works collaboratively with the GMC staff to provide seamless, consistent care for patients around the clock. The hospice has the expertise to manage the complex symptoms and pain-management issues facing patients with a lifelimiting illness, especially those who are particularly fragile. In addition to expert clinical care, Crossroads provides psychosocial, spiritual and bereavement support for patients and families.
